Contact form is designed to gather basic information like your name, email and phone in order for us to provide better customer service to you.

The New Patient & Referral Form is designed specifically to gather name, email, phone, address, point of contact informaton and general provider information.  We do not collect any sensitive, medical, or HIPPA patient information through this form. The purpose is strictly to request a general enquiry for more information about our services. The form is provided for your convenience to reach us electronically.

Google Analytics site tracking is installed on the website, but does not collect any identifiable information from users.  The data is used to view visitor numbers and how the site is used from specific browsers and devices.  The purpose is to determine how well our site is performing.

We do not sell or distribute your information to third-party organizations.  The data is provided to our medical staff in order to provide customer service to you.
